Poland’s top finisher, Dominika Włodarczyk, had to settle for sixth place.
Countrywoman Kaja Rysz finished 10th in the bunch sprint in Chełm, a historic town near Ukraine.
Ahead of the final stage on Thursday, Consonni holds a five-second advantage over Zanetti in the overall standings.
Poland's Rysz sits in sixth place overall, ahead of seventh-placed Włodarczyk, both 16 seconds behind.
The women’s Tour de Pologne is now in its third year.
The three-stage 333-km-long race is being held entirely in Poland’s southeastern Lubelskie province on mostly flat terrain.
Thursday’s final leg runs from the spa town of Nałęczów to Kraśnik, about 50 km southwest of the regional capital Lublin.

On Sunday, American rider Brandon McNulty won the men's 2025 Tour de Pologne, after capturing the overall lead with victory in the final time trial stage in the southern town of Wieliczka.
