Kayaker Anna Puławska took two golds, winning the women's K-1 500 metres and teaming up with countrywoman Martyna Klatt for victory in the K-2 500m, both Olympic-distance events.
Canoer Wiktor Głazunow added a bronze in the men's C-1 5000, a non-Olympic distance.
Hungary topped the nations table with 14 medals, followed by Britain with nine, and Ukraine with five, including four gold.
Poland finished seventh overall.
More than 800 athletes from 71 countries competed in Milan. Paddlers from Russia and Belarus took part under a neutral flag, Polish state news agency PAP reported.
The 2025 Canoe Sprint World Championships took place in Milan from August 20 to 24.
