The lineup announced by Nina Patalon includes Barcelona Femeni striker Ewa Pajor, who recently earned her 100th cap for the national team.
The final squad was trimmed from a provisional 27-player list released a week earlier.
Women's Euro 2025, which will feature 16 teams, marks Poland’s first-ever appearance in the continental championships.
"This is a special moment for the team," Patalon said. "We will do our best to prepare for the tournament as best as we can."
Poland will play a pre-tournament home friendly against Ukraine on June 27 before flying to Switzerland on June 30.
The tournament runs from July 2 to 27.
Poland have been drawn into a group with Germany, Sweden and Denmark, and will open their campaign against Germany on July 4.
Unbeaten in their last 10 matches, Poland recorded four wins last year and five wins and a draw so far this year.
Earlier this month, more than 10,000 fans attended Poland's 3-0 home victory over Romania in their final UEFA Nations League B match, setting a record for women’s football attendance in Poland, according to officials.
Poland topped League B, Group 1 with 16 points and secured promotion back to League A after being relegated last year.
Poland are currently ranked 27th in the world by football's global governing body FIFA.
