Of the new cases confirmed on Monday, 320 were in the central region of Mazowieckie, which includes the national capital Warsaw.
Meanwhile, 241 new infections were reported in the densely populated southern coal-mining region of Silesia.
The western province of Wielkopolskie, which is home to the major city of Poznań, had the third-highest number of new infections confirmed by officials on Monday, at 201.
The latest deaths in Poland’s coronavirus outbreak are 11 people with pre-existing medical conditions and 11 who died directly because of COVID-19, the Polish health ministry said.
On Sunday, Poland confirmed 147 deaths and 3,852 new coronavirus infections nationwide, compared with 421 deaths and 4,765 fresh cases a day earlier.
On April 8, the country reported its highest daily toll of 954 deaths related to the coronavirus.
On April 1, the Polish health ministry confirmed 35,251 new single-day cases, the most since the pandemic hit the country more than a year ago.
Poland's first case of coronavirus infection was reported on March 4, 2020.
15,925 in hospitals, 114,652 quarantined
The health ministry announced on Monday morning that 15,925 COVID-19 patients were in hospitals nationwide, 2,083 of them on ventilators, with a further 114,652 people quarantined for possible coronavirus exposure.
Meanwhile, 2,576,034 people have now recovered from COVID-19 throughout the country, including 6,533 over the last 24 hours, the health ministry also said.
